Information Regarding 1095 Forms

There is a new act called the Paperwork Burden Reduction Act that took effect on December 2024. It eliminates the mandatory annual mailing of Form 1095-B and 1095-C to employees by requiring them to request a copy. You generally do not need the forms to file your tax return, as you can self-attest to coverage. While the information on these forms may assist in preparing a return, they are not required.

If you are wanting a copy of your 1095-C, Unify is CNI's vendor for Affordable Care Act (ACA) reporting and will be responsible for sending copies to employees who are requesting their 1095-C form. Click the files below for more details, 1095 employee request form, and how to access their portal.
